Terms and Conditions

1. This Accu-Chek Smart Pix® Voucher Campaign is operated by Roche Diagnostics Limited (“the Promoter”)

2. This promotion is open to residents of the UK and Ireland only, excluding employees of RDL, their families, agencies, advisors, pharmacies, suppliers and any persons assisting in the promotion.

3. Participants must be diagnosed with diabetes and using an Accu-Chek® Blood glucose monitoring system and be recording their blood glucose levels.

4. Promotion ends 31December 2008. To receive a complimentary Accu-Chek Smart Pix, applicants must go on line at www.accu-chek.co.uk/smartpix in order to complete the registration form in its entirety, including accepting these terms and conditions.

5. This voucher is only available through participating Health Care Professionals.

6. No purchase necessary. Only one Accu-Chek Smart Pix voucher per household.

7. This voucher cannot be exchanged for cash or other goods and services.

8. To the extent permitted by applicable law, in the event of breach of security or fraud, the Promoter reserves the right to cancel or suspend the promotion at any time.

9. By registering for this promotion, participants will be deemed to have accepted and be bound by these terms and conditions which will be governed by English law and be subject of the exclusive jurisdiction of the English courts.

10. In the unlikely event of a dispute, the Promoter's decision will be final and no correspondence will be entered into.

11. All personal data submitted as part of the promotion will be stored and processed on behalf of Roche Diagnostics Limited as data controller in accordance with applicable data protection laws. All such data will be processed within the EEA and will be used by the Promoter, its affiliates and representatives for the purposes of forwarding Accu-Chek Smart Pix to you. The Promoter would also like to use the personal data which is provided by you when registering for the Accu-Chek Smart Pix for any relevant future activities of the Promoter. 12. The Promoter may wish to invite you to take part of a survey for market research purposes.

13. The Promoter reserves the right to cancel or amend the terms and conditions of the Accu-Chek Smart Pix campaign at any time without prior notice. Any changes will be posted on the www.accu-chek.co.uk/smartpix web page.
