Implementation options

We can support the implementation of the below options in the individual healthcare settings depending on local preference and digital readiness.

Implementation in Primary Care

With extensive experience, we will partner with your practice to identify a formulary implementation method that best suit your needs, taking into consideration available resource, compliance with guidelines, patient numbers and the level of support required from us. Implementation tools will vary, but may include:

  • In-practice blood glucose monitoring review

    • Collaborating with you to help plan the review, provide sample text and letter templates, and training where required. Your practice will need to lead the review and invite patients to collect their Accu-Chek Instant meter.
  • Home delivery meter review

    • Partnering with you to conduct a blood glucose meter review, where we send your eligible type 2 diabetes patients their new Accu-Chek Instant meter directly to their home.
  • Use of a third-party diabetes review service

    • Providing an in-practice diabetes review service, conducted by an independent Clinical Pharmacist, that is aligned to NICE guidelines.

Secondary and Community Care

We will partner with you to personalise implementation to meet the needs of your team and the specific patient groups you work with. This includes product training and regular meetings with our Accu-Chek representatives. Additional clinic support is offered when using Accu-Chek Care.

  • Patient usage guide

    • The patient usage guide, sent out with all meters, provides step-by-step instructions to support patients to set up and start using the Accu-Chek Instant meter and mySugr app straight away.
  • Meter registration

    • Encourage your patients to register their meter, by doing so they will also receive a series of helpful onboarding emails to get them started.
  • Online support

    • From FAQs to product manuals, our support resources are here to support your patients every step of the way.
  • Customer Care

    • Our Accu-Chek Customer Care team is ready to support your patients should they have any difficulties using their Accu-Chek products or connecting to our mySugr app.
