Sharing your diabetes related data with your healthcare professional

Your healthcare professional has invited you to share your diabetes related data with them on the RocheDiabetes Care Platform. There are two methods for sharing your data:

  • Option 1 - using the mySugr® app
  • Option 2 - directly within the RocheDiabetes Care Platform

Your healthcare professional will discuss these two options with you and will start the enroll process for the most appropriate method(s).

 

Why share your results with your healthcare team?

You don’t have to do diabetes alone, share your results with your healthcare team. Feel comfortable sharing your diabetes information, fears or worries. They are there to help you. Whether you decide to share your results using the mySugr Reports function, or seamlessly once connected to your healthcare professional’s system†, there’s plently in it for you.

 

No more paper logbooks

By using the Accu-Chek Instant meter and the mySugr app you don’t have to write down your blood sugar readings, so you can say goodbye to your paper logbooks as everything can be stored in one place... your smartphone.

 

Better information, better support1

Having access to your results before your appointment can help your healthcare professional to manage their time with you more effectively, for example, reviewing and adjusting your treatment where necessary. It may also provide you more time to discuss what is important to you.

 

Option 1 - mySugr app users:

How to share your diabetes data using the mySugr app with your healthcare professional on the RocheDiabetes Care Platform

Please note: You should use the same email address for both your RocheDiabetes Care Platform and mySugr app accounts. If you are already setup to use both platforms with two different email addresses, you can either update your email address in the RocheDiabetes Care Platform patient portal or ask your healthcare professional to update your details on RocheDiabetes Care Platform.

1. mySugr app invitation and data sharing

  1. Tap the link "Click here to download or open mySugr app" in the email* inviting you to use the mySugr app to share data with your healthcare professional.
    • For new mySugr app users: 
      • Download the mySugr app from the App Store or Google Play and follow the prompts to create an account.
      • Update your mySugr app Profile & Settings  (units, target ranges, diabetes type and select your blood glucose meter).
    • For existing mySugr app users: 
      • Select Open in mySugr (mySugr app will launch with connection button to connect your mySugr account with RocheDiabetes Care Platform).

2. Connect to the RocheDiabetes Care Platform

  1. Tap Share now and then Connect to share data with your healthcare professional.
  2. Open the email* in your inbox with the subject "Password Reset Request" and tap the link "here".
  3. Select Open in mySugr and follow the prompts to create a password for your RocheDiabetes Care Platform account.
  4. After agreeing to the terms your mySugr account will be linked with your RocehDiabetes Care Platform account.

3. Connect your blood glucose meter

  1. Select your meter from the mySugr app Connections screen and click Connect now. Follow the prompts to connect your meter.
  2. Your data will be imported into the mySugr app and automatically synced with your RocheDiabetes Care Platform account.

How to share your diabetes data using a RocheDiabetes Care Platform account with your healthcare professional on the RocheDiabetes Care Platform

1. Login for the first time

  1. To access your RocheDiabetes Care Platform account for the first time, click the link in the email* from RocheDiabetes Care Platform inviting you to set your account password.
  2. Follow the prompts to set up a password for your account.

2. Verify your account and update personal information

  1. Sign in to your account with your new password.
  2. Read and accept the security consent.
  3. Verify your account by clicking the link in the verification email and sign in.
  4. To update personal information, click on your name in the top menu and then click Edit Profile.

3. Install the Device Link Software

In order to upload data you must download the Device Link Software.

  1. Click on the Upload Data icon to begin to install Device Link software.
  2. In the pop-up, click Download Device Link Software and follow installation steps.

4A. Uploading Accu-Chek blood glucose meter data

The Device Link icon and Accu-Chek 360° Connection Manager icon must both be visible in your system tray / notification area within your taskbar.

  1. Using a standard micro USB cable, connect the blood glucose meter to your computer.
  2. Follow the on-screen instructions to complete the data upload process.

You may be required to sign in to your account to complete the upload process.

4B. Uploading data from non-Accu-Chek devices

The Device Link icon and Accu-Chek Connection Manager icon must both be visible in your system tray.

  1. Connect the USB cable** from your computer to your blood glucose meter.
  2. Right-click on the Device Link icon in your system tray.
  3. Click on Manual Upload - Other Devices.
  4. Follow the on-screen instructions to complete the data upload process.

You may be required to sign in to your account to complete the upload process.

** It is possible that your non-Accu-Chek blood glucose meter may require the use of a special cable from the device manufacturer. Contact the meter manufacturer support team to acquire the necessary cable.
